Consent & waiver

This document is a working draft.
It has been written against how Basemarks actually intends to operate, but it has not yet been reviewed by counsel and it is not yet in effect. Basemarks is not taking bookings and is not collecting customer health information. This page is published so the structure is in place and can be reviewed; the final version will carry an effective date.
This is the plain-English version, published in advance. You will be asked to sign a consent form electronically when you book, so you can read it at your kitchen table rather than on a clipboard in a waiting room with someone standing over you.
What the procedure is
A whole-body dual-energy X-ray absorptiometry (DEXA) scan. You lie fully clothed on an open padded table for approximately ten minutes while a scanning arm passes above you. Nothing touches you, nothing is injected, and there is no sensation.
Radiation
The scan uses ionising radiation at a very low dose. Published effective doses for whole-body composition scans are in the range of roughly 1 to 10 microsieverts — comparable to somewhere between a few hours and about a day of ordinary background radiation exposure. Low is not zero. You are consenting to a small, quantified exposure, and you are entitled to know that plainly rather than have it described only as "safe."
Screening, and why it is not a formality
By booking, you confirm that:
- You are not pregnant and have no reason to believe you may be.
- You have disclosed any contrast dye, barium study or nuclear medicine procedure in the preceding seven days.
- You have disclosed implants, surgical hardware, or retained devices.
- Your screening answers are accurate to the best of your knowledge.
We will ask you again in person on the day. If anything has changed, say so — there is never a fee for a cancellation on safety grounds.
What this service is not
You acknowledge and agree that:
- Basemarks provides measurement for wellness and fitness purposes. It is not a diagnostic service.
- Your report is not a diagnosis, and no diagnosis will be made or implied by our staff.
- A whole-body scan is not a clinical bone density study of the hip and lumbar spine, and does not substitute for one.
- Nothing in your results should be used to start, stop or change medical treatment without consulting a qualified healthcare provider.
- Measurements carry inherent precision limits, and results from different scanners are not directly interchangeable.
Clinical authorisation
Scanning at Basemarks will be performed under a written standing order authorized by a licensed Georgia physician, and the screening questionnaire will implement that order. This document will be updated to name the authorizing physician before any booking is taken.
This arrangement governs the conditions under which we may scan you. It does not create a physician–patient relationship between you and that physician, and it does not entitle you to a clinical consultation, examination or diagnosis.
Your information
You consent to us collecting and using the information described in our privacy policy, including your screening answers and scan results, for the purposes set out there.
You are not consenting to any advertising use of that information, because we do not make any. We do not sell it and we do not share it with advertisers.
Text messages
Appointment reminders by text are optional and consented to separately, on the booking form. You can opt out at any time without affecting your ability to book. Standard message and data rates apply.
Photography
We do not photograph or record guests. If we ever ask to — for example, for a testimonial — it will be a separate, explicit, entirely optional request, and no is a complete answer.
